1. Patient Consent and Agreement
You acknowledge and agree to the following terms and conditions for receiving care from Harmonia Women’s Health, including via telehealth.
2. Scope of Care
Harmonia Women’s Health provides specialist care in women’s health, focusing on perimenopause and menopause. Our services may be delivered:

In person at our clinic,

Via secure telehealth platforms, including phone or video consultations.
You acknowledge that telehealth consultations may involve limitations in physical assessment. If necessary, you may be referred for in-person review or additional testing.
Harmonia Women's Health is not a General Practice, and its scope of practice is limited to Menopause. Harmonia Women's Health clinicians will not treat or advise on any unrelated medical conditions.
3. Consent to Telehealth
You consent to:
Receiving healthcare services via telehealth (phone or video) if applicable.
The exchange of your personal and health information electronically for the purposes of diagnosis, treatment, follow-up, and coordination of care.
The risks associated with telehealth, including potential limitations to confidentiality or data breaches (though we implement industry-standard protections).
4. Fees and Cancellation policy
We are a privately billing practice. Consultations (in-person and telehealth) attract a fee. Current fees are available on our website. A deposit of 50% of the fee is payable at booking and credit card details are securely stored. Online credit card fees apply. The remaining 50% is debited after the appointment and your Medicare rebate will be processed if applicable.
Please note that fees are charged according to actual appointment time not booked time.
Appointments can be rebooked or cancelled up to 24 hours prior to your appointment without charge. Appointments cancelled after this time will incur a cancellation charge of 50% of the scheduled fee.
In cases where a Telehealth service may attract a Medicare rebate which is to be bulk-billed, eg. short phone call regarding results, you agree to assign your right to benefits to the practitioner who has rendered the service.
5. Privacy and Health Records
We collect, use, and store your personal information in compliance with the Privacy Act 1988 (Privacy Act) and the Australian Privacy Principles (APPs). This includes:

Your name, date of birth, contact details, medical history, and treatment notes,

Information shared with other healthcare providers involved in your care (e.g., GPs, specialists, pathology or imaging services),

Your medical information will only be stored on secure, cloud-based medical software.

6. Sharing of Information
